Enrollment

Prior to acceptance into the daycare center or preschool, the parents need to enroll their children.  The enrollment procedures include the completion of forms, a tour of the facilities, the Parent Handbook read and signed, copy of immunization records given to us, references checked, and the supply fee paid (3-12 year old side only).

The forms may be picked up at the daycare room: 410 Collins Ave. Mandan, ND.  The forms that need to be completed prior to the child's first day are:

1. Enrollment Form
2. Health Assessment Form
3. Billing Plan Form
4. Immunization Form
5. Free or Reduced Hot Lunch Form
6. Parent Handbook Agreement Form
7. Supply Fee paid
